New Patient Information

• New patients are always welcome at our office. We proudly offer new patient appointments within 48 hours of your call. Because we understand your time is important, we try very hard to make sure you are seen as close to your scheduled time as possible. We strive to make your wait time as little as possible.
   

• Our office staff is friendly and qualified to answer any of your questions.

• Download our "New Patient Information Form" and "Medical History Form" and fill them out at your convenience prior to your first appointment to reduce your check-in time. Simply click on the link (title of the form) and print the pdf file.
   

• We utilize the latest equipment and sterilization techniques. We offer lasers to aid in diagnosis of early cavities as well as oral treatment. We feature ZOOM! In Office Whitening and ClearCorrect, a clear alternative to braces.

• Upon your visits, should Dr. Miller find a problem, a treatment plan will be provided that will outline the total cost and estimate what your insurance will pay. It is our policy that your portion of the treatment cost will need to be paid at the time the treatment is scheduled.
